After Hospital Support/Transition Care

 

Scout Nursing provides Community nursing services to all transition NDIS participants (renal failure, Cancer, Heart disease, Cystic fibrosis, dementia, end-stage liver disease, kidney failure, lung disease, Parkinson’s disease, stroke). ​

Our After Hospital Support/Transition Care services are designed to provide comprehensive assistance and continuity of care for NDIS participants transitioning from hospital settings back to their homes/community.

Rehabilitation Services:

Home Health Monitoring:

Assistance with Activities of Daily Living (ADLs):
