Scout Nursing provides Community nursing services to all transition NDIS participants (renal failure, Cancer, Heart disease, Cystic fibrosis, dementia, end-stage liver disease, kidney failure, lung disease, Parkinson’s disease, stroke). ​

At Scout Nursing, we understand the critical importance of a smooth transition from hospital care to the next phase of recovery/continuous care. Our After Hospital Support/Transition Care services are designed to provide comprehensive assistance and continuity of care for NDIS participants transitioning from hospital settings back to their homes/community.

Post-Discharge Assessment:

Our experienced healthcare professionals conduct thorough assessments to understand each patient's unique needs, ensuring a tailored approach to post-hospital care.

Personalized Care Plans:

We create personalized care plans that address support needs. These plans are developed collaboratively with the patient, their family, and the healthcare team.

Medication Management:

Our team ensures proper medication adherence, including organizing medications, providing medication reminders, and coordinating with prescribing physicians for any adjustments.

Rehabilitation Services:

We offer rehabilitation services to support individuals in regaining strength, mobility, and independence. This may include physical therapy, occupational therapy, and other specialized interventions.

Home Health Monitoring:

We provide regular check-ins to track vital signs and overall health progress, allowing for early intervention if needed.

Assistance with Activities of Daily Living (ADLs):

Our caregivers provide assistance with daily tasks such as bathing, dressing, meal preparation, and other activities, fostering a comfortable and supportive environment.

Coordination with Healthcare Providers:

We facilitate communication and collaboration between the patient's primary care physician, specialists, and other healthcare providers to ensure seamless and integrated care.

Emotional and Psychological Support:

Our team provides emotional and psychological support, including counseling services and resources for mental well-being.

Family and Caregiver Education:

We offer educational resources and training for family members and caregivers, empowering them to actively participate in the ongoing care and support of their loved ones.
